Your participation in this survey is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time. If you decide not to participate in this survey or if you withdraw from participating at any time, you will not be penalized. If you choose to participate in the sweepstakes portion of this survey, your email will be collected for the sole purpose of notifying the winner. Four winners will be chosen at random to receive an Amazon gift card. Participants must be over the age of 18 to collect the prize.
